Jotform is appearing on my credit card statement mutlitple times. Just received another email that we've been upgraded again to premium status. order #INT140625-4711-75978B Per our records we should be paid through 8-24-14. Wondering what is going on, as this was set up for us to pay monthly as needed, not as an automatic charge to our credit card.
Please contact me so this can get straightened out.

It appears you may have accidentally signed up for two subscriptions, one was started May 19th 2014 and one was started May 25th 2014. I have cancelled and refunded the one that originated on May 25th as well as the second double payment made on June 25th 2014. You should see this reflected in your account in 5-7 business days. I also went ahead and cancelled the duplicate subscription. You should now only be billed for the one that is on the 19th of every month.
