The autoresponder is going to go to your own email address if you have filled in the form to test and submit it to yourself. The notification on the other hand will go to the form owners email address unless you have set this up to go to a different address such as your own if the same.
When I checked your email notification for the above form I found there are 3 email addresses listed. I checked all of these in our system, but these are not in our bounce list. I would also advise checking your spam folder to see if maybe they went there otherwise you will probably need to whitelist jotform ip addresses with your email host since you are using a custom emails. Thank you so much for looking into this.
I checked my spam and nothing is in there. I do get test emails and confirmation (when I put my email address). I still do not get notification emails. I removed the other notification recipient email addresses and put mine only - still do not get notification email.
I use JotForm and lot and have created many form but it's only with form that I'm having issue.

I see your email notifications is correctly setup:
Please white-list our domain names if the issue persists. (We use Amazon SES method for "email@example.com" sender.)
