but i can not submit my form.

Your account is indeed well below your monthly limits and the form has not been disabled on our end. It does appear that you may have accidentally disabled it on your end:

You did not received the email notification from the form http://www.jotformeu.com/form/21116720181340 because the recipient email was in the bounce list due to the following reasonResult: firstname.lastname@example.org is IN the bounce list
Reason(s) : X-Postfix; lost connection with 126.96.36.199[188.8.131.52] while receiving the initial server greeting
I have now removed the email email@example.com in the block list. Incoming new email notification should now arrive in your email.
