That form is currently disabled - and hence you cannot receive any email notifications.
I'm not sure what caused it to be disabled - but....
To enable, please follow the steps below:
I checked our bounce list for your email - no problems there - the email notifications should arrive as expected once the form is enabled.
Please let us know if you experience further difficulties.
