There are a few things I can think of:
If you make changes and click preview without first clicking on save, it may be loading the saved version rather than the altered one. You may also be loading a cached version of the form. I would recommend clearing your web browsers cache and history to see if that clears it up. The last thing I can think of is that you may need to clear your actual forms cache. Here is a guide on how to do so:
