Why is my account suspended?

Need to get our account reactivated immediately, we have several clients' forms on here and they are extremely unhappy that their forms are down. What is the issue???

The form you have entitled "Sample Sign Up Form" set off our automatic phishing detector and you may want to remove it as it could do this again.  I activated your account but please refrain from making forms that ask for a large amount of sensitive information as well as any user account details.  Specifically asking for usernames and passwords is against our terms of use and will cause your account to be automatically suspended by our filter.
