Some forms that are posted to us have never arrived. Sometimes the people who have posted forms to us cannot send e-mails to us or receive e-mails from us.

It sounds like the emails are getting stuck on your or your customer's spam filter. My suggestion would be to use a different email address for a while and see if it helps.
