Conditional drop down boxes

    Is there a way to add a drop down box and based on the individual's selection, generate a pop up window or additional (previously unseen) fields that request additional information?

    I would like to add a section to our donation form that asks if the donation is a) in memory of or b) in honor of someone.

    Based on what they select, I'd like to then have them fill in fields of who the donation is in honor of/in memory of and if and where the notice of donation should be mailed or emailed.

    Yes it is possible to achieve your requirement. You can take advantage of our "Condition" feature to show hide form fields based on the input of form fields. The following guides should help you :  http://www.jotform.com/help/57-Smart-Forms-Using-Conditional-Logic

    If you want to show / hide multiple form fields based on one condition, you do not have to add multiple conditions or action items but you can simply show / hide them in one condition. The following guide should help you:  http://www.jotform.com/help/196-Hide-or-Show-Multiple-Fields-at-Once
