Hi, I am really sorry to post here, but I am getting a bit desparate.
I am a premium user but I am getting no response to my emails sent via (http://www.jotform.com/contact) from Jotform. I have now sent 4 messages on 180710 / 2 on 130710 (0248 & 1817) and 1 on 080710. At one point I was aware that you had tested our form, but there was no fix and no contact from you.
The problem I have is with blank name and phone fields on all my submission notification emails. This seems to be a bug. I have checked and double checked the HTML and the field codes multiple times but the issue is still present. All other fields are fine. We needed to get this form live days ago but can't because of this issue.
Please can you respond.

Sorry just to correct my 4 messages were actually sent 150710 / 2 on 130710 and 1 on 080710 - I haven't (as yet) sent one in the future.

Just to add in - there is also an issue with the Title field in our actual submission reports - the only selection that seems to register is Miss, any other selection is simply not reported at all.
Is there anybody there?

OK - as I haven't heard anything I have had to recreate all the fields - thus losing data from previous entries in old fields. I had backed this info up, but it is now inconvenient that we will always have to patch the original data captured in to the new excel submission file.
It seems to be working right now. Keeping everything crossed that this bug doesn't reappear.
