I checked the form in your website http://www.orthoticsforyou.com.au/contact-us/ using Chrome browser, but I was not able to reproduce the problem.
Can please tell us the Chrome version the browser where you have see this problem. We will check further if issue still persist.
Yes I saw you submit that, thanks. However I changed out the form to the above temp form while I waited to hear back from you.
The template I have the problem with is "Contact Form With Blue Envelope Theme". It's 100% a problem with it not displaying in Chrome correctly now. I tried on multiple PC's. It worked fine in IE.
Do I need to swap the broken form back in order for you to look at this or can you load that template with Chrome yourself?

Can you create a new test page such as http://www.orthoticsforyou.com.au/contact-test/ so that we can see the problem without disturbing your live site? Please let us know the URL and we will be able to tell you what exactly might be causing this.

I've just tested again and it works fine in IE v11 however it is still cut off in Chrome v35.

I was able to see the problem of a cut-off form in your test website.
To fix this, please try re-embedding the form in your website using its iframe embed code.

Please inform us if problem persist even after doing this already.
Thank you so very much for your help. That has fixed it now. I'll remember not to use the wordpress version of the code and also to update the code if I ever change the form in any way.
