How can I have my forms enabled again?

Please help. I was just working on forms on my account and it appears they have all been disabled.  How can I get them enabled again?

Hi,

We do not allow forms that collect login information.  Your Form title "Material Brokers Client Login" asks only for this information and set off out automatic phishing detector.  I went ahead and activated your account but I would strongly recommend removing that form as it may cause your account to be suspended again.  Please let us know if there is anything else we can assist you with and we will be happy to help.
