I have finished it but when I ask for preview, I see that the button radio front of each item going wrongs.

In order for us to investigate your concern further, please upgrade your account into a Free account by registering to JotForm. Just go to our sign up page and register using your email address firstname.lastname@example.org so that your form will be carried over into your new account. We'd like to see how your payment form is set up. Currently, your account is a Guest account which means you have not registered for a JotForm username and password yet.
