Problem with my form

    Dear Sir or Madam

    Could you have a look on my form here: http://french.jotform.com/form/12330135614

    And say me what is going wrong with it?

    I have finished it but when I ask for preview, I see that the button radio front of each item going wrongs.

    In order for us to investigate your concern further, please upgrade your account into a Free account by registering to JotForm.
