Could you please help me get my notifications to come to me?
See the "Apply" button for the form.
And the notifications for the quiz form are not coming to me.

I have a hard time opening your website. Anyway, I checked your forms and your notification email settings seems to be fine. They are all configured correctly.
I also checked our email logs and it shows that all form emails were successfully delivered your custom email address:

Did you check your spam folder? Are you using an email client for reading your emails? If yes, I would advise to check emails using your webmail access - if you have an access to that.
Furthermore, since you are using a custom email address, we advise adding our IP Addresses and Domains to your server whitelist. You can find all the list here: http://www.jotform.com/help/145-Whitelisting-JotMails-IP-Addresses
Alternatively, you can also use Mandrill's SMTP to send emails from your form. This guide: http://www.jotform.com/help/236-How-to-use-Mandrill-to-Send-Emails-From-Your-Own-Email-Address.
