    Hi,

    Can you please try re-embedding the form using a fresh iframe code. 

    Follow this guide  -Getting-the-Form-iFrame-Code

    When I check your form http://www.jotform.us/form/41462019561147 in your website, I see this problem.

    Re-embedding the form using its iframe embed code should fix this problem.

    Hello ldsrhb,

    I am not sure if I have understood your question correctly.

    I did try to check your web page where you have embedded your web page but it seems to be password protected. 

    To check it how it looks, the only option is to grab your form's iFrame embed code and then embed it in your web page.
