How can i get a waiver to be input into the PDF created with complete forms

    I have a few things im needing to do with our waiver.

    I need our waiver to show up and have a link in the form so the client has easy access to it (legality of release) but when the email notification is received the waiver cannot be seen. I have put it into the HTML section but reading the forums have discovered that wont do it.

    My form is linked to a google drive so all completed forms are saved as PDFs, i need the waiver to be apart of that so if, our lawyers need a copy of the form for any reason the waiver text is all completed and with the form answers/responses.

    My lawyers have told me it all needs to be saved into the one doc as the waiver being separate is not legal if we are sued they can claim its not the same waiver release they signed.

    Hello Philip,

    Please be noted that the headings and the images cannot be displayed in the submission PDF file which gets uploaded to integrated google drive. 

    There is an another way to achieve your requirement. You can add a simple checkbox question and add the content of "Risk Waiver Release" there. Please check the screenshot below:

    Having the content in a field will ensure that it gets added to the notification email as well as the PDF document as well. Please check the screenshot below on how the text of "Risk Waiver Release" will be displayed in the email and PDF:
