We suddenly stopped receiving email notifications for the above form. I have checked all spam filters and email server logs. It's supposed to send to the email address of "email@example.com"
I do not have SMTP Authentication turned on for this email address.

Upon checking your form's notification email alert, I found that you have set "firstname.lastname@example.org, email@example.com, firstname.lastname@example.org" as the "Recipient Email" address. It is not sending submission emails to "email@example.com" email address. Please check the screenshot below:
I did check our email logs and found that submission emails are being sent to all the email address you have added. Please check the logs below:

I would suggest you to add the email address "email@example.com" in the "Recipient Email" address of your email alert and then test your form.
