How can I create a table

I would like to create a table where my client can select the appropriate price based on , the size,  and number of subjects.  I have enclosed a sample table

Hi, please try using the Multiselect Grid widget, in this way if you have options that can be layed out on a grid, your form users can select each option by clicking a cell on a grid. Also try the Data Grid widget.
