How do I upload a pdf file that I link to in a form?

If I understand your requirement correctly, you want to display a pdf document in your form. Is that correct?

You may like to add a "PDF Embedder" widget in your form which will display the required PDF file. You will need to provide your PDF URL along with the height/widgt.

Please be noted that PDF needs to be uploaded in your own server as you can just input the PDF file URL but cannot upload the file directly.
