How can I enable multiple selections in my form?

Since my questions has one or more answers ( Eg- Radio buttons cleared all other options)

I just wanted to allow user to select multiple options.

i'm working for NGO, and i have made few surveys questions regarding children health.

Hello abctrust,

To be able to allow your users to select multiple options, you should replace your radio button question with check box questions from "Form Tools" tab. Having check box question will allow you to select multiple options from the list. Please check the screenshot below:
