For some odd reason my date/calendar field is freezing. Whenever I select the date collected field to enter a date the calendar remains visible hiding the the date reported field.
It was my thought that maybe I could delete the two fields and replace them with the same fields, however, I am getting the error message that I will lose all data associated with this field.
Please advise on how to resolve this issue ASAP.

Thank you for reporting. I have now raised this to our developers so they can check and investigate this issue further. You will be informed here once this is resolved.
While waiting for the actual fix, I would suggest using a cloned version of your form. It should be working. Here's how to clone your form. http://www.jotform.com/help/27-How-to-Clone-an-Existing-Form

Thanks but if the form is cloned, how will I be able to have all data in one form. We are interested in keeping all the data connected to one form.

You can use this app to import form data http://apps.jotform.com/app/import_data to your cloned form (and here's how to download form submissions to your other form), but I don't think that's necessary.
One of our developer is already looking into this issue. We will make sure to inform you right away once this is fixed.
