Form Collapse does not work with Mozilla/Firefox

Hi,

I have a form with collapses that work fine with Safari but do not work with Mozilla/Firefox. 

I've had to temporarily redo the form to remove the collapses so the form can be submitted.  Do you know what the issue is? 

Again, the form works fine with Safari.

Can you please clone a form, and add the Form Collapses to that cloned version? We will need to test the form to be able to provide you with additional information on the issue.
