I have a site that consistently has a form that needs the code dropped back in over and over again.
I have moved on from development of the site, yet this keeps happening. Please help!

Something else might be overwriting that file. Possibly when you upgrade your wordpress. You should probably contact wordpress support forum and ask them why that particular file is getting overwritten. Maybe they suggest a solution.

This happens to me as well. It actually happends when I'm editing the page that the embed form is on. I drop the embed code in the html tab and then if I click on the Visual tab, the embed code will be gone and replaced with:

I see what you mean here. You should not switch it to visual tab after adding the codes on HTML view or else your embed codes will be deleted. I believe this is a problem with wordpress.
If you'd like to add extra words along with JotForm, I suggest to write it first on visual tab before embedding jotform code to HTML tab.
