e-mail setup dialog boxes are blank

    E-mail setup boxes are blank, so I can't read it.
    When I click the options it goes to the next step, but I'm not sure what is it about.

    Please try to save the form, refresh the form builder. See if that will resolve the issue. If not, update us here.
