Order form with multiple options

I'm trying to create a t shirt order form that allows for multiple variables such as type, gender, size, quantity, etc.  For example, I want to allow people to enter that they want 2 Large Adult t-shirts, 7 Medium Adult t-shirts and 4 Small Men's Singlets. In addition to the variables, it would be nice to have a field that calculates the totals of each variable.  Is any of this possible?

Do I have to attach payments to it? I don't have dollar amounts as we're not "selling" these.

From what I understood you don't want to collect payments via form. Is this correct?

If you do not wish to have payments you can use a Purchase Order instead and add "No currency" option:
