Login/Password

I don't suppose you have widget or app that allows users to only access the form once they have entered a user name and password?

We don't create login forms that is against our Terms of Use policy. It is considered phishing when creating a login form. If you aim in creating this kinds of form our automatic phishing tool will detect these form and may result your account being suspended.

What you can do is use a conditional logic and set a certain key for user to input before the fields will be shown to them.
