Dual Repsonse Types

Is it possible to have a response type be a text box and a file upload?  i.e. Question is to respond or upload a document

Yes this is possible if you use conditionally hidden fields:

Depending on what you are trying to accomplish, you could either have a field that ask's a question and shows either the text box or the upload field and hides the other, based on the answer the question.  You could also have the text box hide if the upload field is being used or vice versa.  This would only allow your users to use one field or the other, based on your conditions.
