Using your new Preview Before Submit displays other fields rather than just what the user submitted. I used the widget's wizards to answer NO to "Include Empty Inputs"
Did I miss something in the configuration?

I'm sorry for the trouble caused to you.
I did check your form and found that you have added the "Preview Before Submit" widget twice and that is the reason why the "Include Empty Inputs" are not working as expected. Please check the screenshot below:
Please remove one of the widget from your form and that should solve your problem. It will not display the empty fields anymore. I did clone your form and tested it by removing the widget and it seems to work as expected.
There seems to be an issue with the payment question where the preview widgets displays some IDs. When the product has special pricing it does not get displayed correctly in the preview window. I am reporting this to our back end team. We will get back to you as soon as this issue is fixed.
