Customizing the Recipient of a Form for each Submission

    Hello-

    we would like to create a form for our employee recognition program where supervisors can nominate one of their direct reports to receive a monetary reward. Since the nomination must be submitted to a manager for final approval we are wanting the form to be submitted directly to that manager rather than the Jotform admin who will then need to forward it on to the manager (we are trying to avoid multiple steps in the process). We have hundreds of managers and supervisors (who change regularly) so it would be ideal if an email address can be entered where the form gets directed to.

    So basically I am asking is if there is a way to customize the recipient of the form upon the click of the "Submit" button.

    Hello Cynthia,

    Thanks for reaching out to Jotform Support. I believe your requirements can be achieved by adding an approval flow to your form. Once the user submits the form, the approval flow will send an email automatically to the email of the approver for the approval process. Let me show you how to add it to your form:

    1. In Form Builder, click Settings.
    2. Under the Approval flow tab, click Create an approval flow.

    3. Once directed to the approval page, click the gear icon on the approval.

    4. Under approval settings, set the approver email to where you want the approval to be sent.

    You can also modify the email content that will be sent to the approver by clicking the Edit button next to the Customize Approval request Email feature.

    thanks for your fast response to my question. This method requires that the approval email address be determined when the form is created (and the field is only allowing one email address) but this is not possible for what we need to accomplish. We are a manufacturing facility with hundreds of managers (the approvers) and hundreds of supervisors (the ones who will be submitting the form) and they change out rather frequently. I need each supervisor to be able to submit the form to their own manager, who ever that may be, i.e. type in the email address and the Submit button sends it to that person. The Approval Workflow method still requires the form to be submitted through a single point of contact.

    Unfortunately, the only way to achieve your goal is to create an approval and let the supervisors fill out the form to be directed to their designated managers.

    May I know what exactly you wanted in the form builder itself?
