Per instructions in the FAQ, please check into this and send email logs. The URL for my form appears to be:

Unfortunately, there were no logs to your form's recipient email address upon checking. Please check this guide on how you can fix it: http://www.jotform.com/help/208-How-to-setup-email-alerts-to-prevent-email-bouncing-related-issues. We recommend using Mandrill's SMTP if email@example.com is not working. Whitelisting our IP Addresses and Emails should help as well.
