How can I change the questions on the course evaluation template?

I need to edit my course evaluation form template. Can you please tell me how I can do that?

You have to cloned the or use the template then once it is in your forms you can start editing the form.

Thanks for the reply, I've cloned the form and clicked to edit but its the matrix that I can't amend the detail on.  

You have to click on the Matrix field, in the field properties, you should be able to change the options to the Rows and Columns:
