How come I can not view my form or even view it on my webpage niether of them

However, the strange thing is I could not view your form fields when I preview your form here though, I can see from its source code that there are fields in it.

That usually happens when there's a conflict on the HTML code or if it's messed up. I believe it is something to do with your text fields. Usually, when you copy text from a different source, the HTML formatting will be carried over. This might be the reason why there are jumbled HTML codes on your text fields. The best way to resolve that is to copy the text to notepad first to clear its HTML formatting before pasting it to the text field on your form.
