I have created a form that I have been trying to put on my Facebook page. Every time I click on the "add to facebook" button, I get an error. I have seen on your facebook page that this has happened before. Is there a way to get around this so that I can use my form?

What error message you get when your try to add your form in Facebook?
Have you already checked our guide on how to embed your form in Facebook? Please check the following guide and that should help you: http://www.jotform.com/help/22-Adding-Form-to-Facebook-Pages

I have tried these links. Once I get to 4 in the first link I get an error message on the page. I never have the option to choose the correct fan page.

What error message are you getting? Are you getting an error message something similar in screenshot below:
If you see this error message that means you have not added any page in your Facebook account yet. Please be noted that form can only be embedded in a Facebook page. Please add a facebook page and that should solve your problem. Please check the screenshot below on how to add facebook page:
Once you have added a page in your account, embedding form should be simple and you should not see the error message anymore. Please check the screenshot below:

I am not getting that error. The error that I am getting says that "An error occurred. Please try again later."

I am also unable to reproduce your issue. I added a form to a facebook page following our guide without a problem:
Occasionally this happens when there is a timeout on Facebooks end. Also, make sure you are logged in to both your Jotform and Facebook accounts in the same browser. If you still cannot get this working, I would suggest trying a new web browser. If this problem still persists, let us know and we will be happy to look into it further.

I have been trying all day to get this on. I get the same error every time no matter what web browser I use.

There is a change that this might be a browser related issue. Can you try to add the form to your Facebook page via a Google Chrome browser?
Note, that at this time you can have only one JotForm Tab per a Facebook page.

I have tried to do this again today. I have tried this in Internet Explorer, Google Chrome, and Mozilla Firefox. I still can not get this to add a tab on my facebook page.

That is strange.
Please share your facebook page where you want to add your form and we will take a look if there is any problem.
Which operating system are you using anyway?

The facebook page that I would like to post this to is www.facebook.com/AndiamoTravelConcierge. I am using Windows 8.

I had another look and I am still unsure why this is happening. I tried a few things to see if I could somehow break the integration but it seems to be working on my end no matter the setup. I have opened a ticket to our development team to see if they may be able to assist you further. Sorry for the inconvenience.
