We have 4 items. If my customer chooses one of the last two (in addition to either of the top two), he will need to fill out an additional form. How do i set that up?

Thanks for asking. Since you are using Paypal as payment gateway for your form, you can assign what is called a "Return URL" in your Paypal account wherein once a customer submits payment in the Paypal redirect page, they will automatically be redirected to this "Return URL" which could be another JotForm form or your website's main page.
To set up this Return URL in your Paypal account, log in to your Paypal account and the click Profile link. Then click My Selling Tools on the left side menu. Finally, click the Website Preferences Update link.
This will lead you to the Website Preferences Page where you can assign a Return URL:

By the way, this Return URL will be applied regardless of the item chosen in your original JotForm payment form. My suggestion therefore is to place a note in the Return URL form that "if you did not purchase <name of item>, please disregard this form.

Perhaps if we see what the other form looks like we might be able to come up with alternative solutions.
