How to make subject lines unique?

All my forms have the same subject line. The issue I have with that is that different submission can get bundled in one conversation when they are received in gmail.

Is there a way to make the subject line unique to the submission by including the time and date it was submitted / information received?

I see that you have added Form title and name in the subject line in which duplication of name could be shown under one conversation. 

I would suggest to add the very unique field which is submission id to the subject line as shown below :
