Install form on wordpress site

I have been trying to install a form from my account onto a Wordpress site. I have tried the script for Wordpress that you supply and have also tried the plugin. Only a portion of the form will appear. You can see it here:

Need help...What am I doing wrong?

I checked your form and it seems you have expanded the text box:

Shrinking that should collapse your form back down to where everything fits.
