When we fill out all fields and submit the form, the email we receive through Jotform only shows the name and the email... no other fields are captured - at least not in the email we receive. I am at a loss for what the solution may be... any help is appreciated. Thank you. Michael.Page URL:

This is because only the first 2 fields are configured in the email alert.
To resolve the problem, simply delete and rebuild the email notifier, by clicking on the Delete E-mail button
