How to fix a suspended account?

    This is my first time using jotform, just created an account and after a few times logging in it says the account has been suspended.  "Due to terms of use violation, your account has been suspended.It was suspected to be used for phishing. If you think this is a mistake please contact us to resolve this issue.”  Don’t know what happened or how to fix the issue. Please help, thanks!

    Hi Tessa,

    I have re-activated your account. You have been auto suspended because of a cloned form which contains a login fields (username and password). Our system filter is designed to capture these type of fields and auto suspend the account that the form was created.

    This just happened to me.  How quickly can this be fixed and how do I make sure this doesn't happen again?
