Seeing a blank registration page (was working earlier today)

    From the main URL..

     

    select Provider page and click on the Register link.

    Hi djwoodbury,

     

    I was able to replicate the issue you reported. May we ask which webpage editor you're using? In any case, please try to use the Iframe Embed Code instead of the one you're using now in order to resplve this issue.

    To get your form's iframe codes:

    1.  Go to Setup & Embed tab
    2.  Click Embed Form
    3.  Click iFrame


    4.  Copy the codes provided in the succeeding screen
