I was unable to find any forms associated with your email address so I could not personally test your forms. However if you are having formatting issues with your form, you can try adding our mobile responsive widget:
or customizing the mobile formatting yourself as discussed in this thread:
If your form is embedded to a webpage, usually using the iFrame embed method solves issues with the form not loading at all on mobile:
