Need to switch the form on the Facebook App to a custom form

Need to switch the form on the Facebook App below to a custom form within our account. 

I understand that you want to change the form on Facebook with another form in your account. 

1. Login to JotForm account

2. Login to Facebook account in other tab and go to the page where the form is embedded.

3. You can see the Setting symbol to expand and show the forms in your JotForm account

 

4. Click on the form you want to display and save the page.
