I am able to submit the form successfully using the URL provided and using direct form URL as well.
It seems the issue is with browser cache. I request you to clear the browser cache by following the steps provided in

Thank you so much for your reply, I have remedied the situation. The problem was with the terms and conditions check box. I removed that and found an alternative and we're in the money now. Cheers for a quick reply.
