Are the forms fillable via smart phones or do you need special application?

I managed to setup my form into my website , but when I use my smart phone to view the form on my website it is not there

I am able to replicate the issue you are having. To solve this problem, I would suggest you to embed your form using it iFrame embed code. The following guide should help you grab your form's iFrame embed code:  http://www.jotform.com/help/148-Getting-the-Form-iFrame-Code

Additionally, I would also suggest you to add "Mobile Responsive" widget in your form which will adjust the width of your form fields to display them without any horizontal scroll in mobile devices.
