Can you assist in transferring a form from one account to another?

I would like to transfer the form FutureFavor (http://form.jotform.us/form/40860228327150) to username tdvh56. Can you please help me with this?

Hi,

We can indeed move the form for you with submissions intact, however the request must be made from the account associated with the form.  Please log in to the account you would like to have the forms moved from and create a new post from that account stating which forms you would like to have moved and the username of the account you would like to have them moved to.  Once you do this we will be happy to move them.

This is the account I would like to have it moved from. It is going from username webmosa to username tdvh56.

Sorry, I misread that, I though you wanted a form moved from the account FutureFavor.  I went ahead and moved form ID#40860228327150 from your account to "tdvh56".  You should now be able to use that form as normal from that account.
