One of our forms has suddenly started giving a wrong submission error message

Our probation review form works from our team to line manager, but when line manager tries to submit back to People and Culture, they receive an error message stating "oops, wrong submission ID".  I cannot see that any of the conditions have changed or understand why this has suddenly happened.  Please could you shed any light?

The space after {id} &mode. Remove the space and it shoulde be like this {id}&mode. It's in your Email to LM notifier settings.
