Creating a checklist with a textbox option.

Hello,

I am trying to create an event checklist. I would like to be able to check off an item from a package and then beside that item a text box with a description. I have been trying to use the configurable list widget but it does not look as clean as I would like it to.

I would really appreciate some assistance and if there is another way I can accomplish this.

Thanks for reaching out to Jotform Support. I think the configurable list widget will be a good fit for your requirement if used properly. Looking at your form, I see it needs some customization for the configurable list widget. So, please make sure you follow this guide to set it properly and then let us know how exactly you'd like it to be in terms of appearance and we will provide the needed CSS code for it.
