Add Pretyped Characters/Numbers Before User Inputs

1- under the section asking DO YOUR BILLING ADDRESS IS DIFFERENT THAN SERVICE ADDRESS ?

do i able to have option saying YES and NO . and if they click on YES then another section to fill up billing address open for them otherwise move to next question please ?

2- under the section asking for Tax ID can it be combination of text and number not just number ?

3- i am asking a number, how do i able to already have a pre-typed ( number or letter )something before the number they will fill up always that letter or number or code be there

4- on the section of phone number how do i make it only one area and no restriction of have to have area codes , also when they type the number automatically type in format of (###-###-####).

i do appreciate your help . i watched few youtube videos ,but they are old or i cann ot figure out how to do it i aill greatly appreciate you if you able to fix those request on my form please . would you ?

Thanks for reaching out to Jotform Support. Yes, all those requests can be achieved. To have the yes and no option, we only needs to add it in the Options tab. Let me show you how:

• Click on the gear icon
• Select Options
• Insert the Yes and No

We have checked the Tax ID and it is a Number field

We will need to replace it by a text field so it can accept letters and numbers

Regarding the phone, yes it possible to have it as ###-###-#### format. Let me walk you through it:

• Click on the Gear icon
• Select the option tab
• Turn the Input Mask ON
• Customize the format as you want

On your question 1 and 2, you may like to take a look at our "Input mask" feature which will help you achieve your requirement. You can set the first part of the inputs to text and the following will be numeric. Please take a look at the screenshot below. Additionally, you can check this guide for more information.
