Can you please tell us your JotForm username? Or the URL of your form..
I am not able to find an existing account using the email address you had used on this post.
I also could not find a JotForm form in the website you had mentioned.
In any case, you can request for a password reset using your email or username by following this steps.
1. Go to JotForm Login page https://www.jotform.com/login
2. Click on the 'Forgot Password' link
3. Provide your JotForm email account or username and click on submit reset button.
