Form as guestbook

Also, do you have any forms that will serve like a guestbook? Would like to be able to have it so that users can leave comments on the site and then have those comments visible on the site.

Unfortunately, we do not have a direct feature that would accomplish this one. A workaround would be using our Facebook Comment Widget to your form:
