added Mobile Response widget, still doesn't work, what's wrong?

I added the Mobile Response widget as suggested, re-embedded the code but it still doesn't work right on my iPhone. what did I do wrong?   I added the widget at the top of the form, I did not alter any of the settings in the widget. I need help, please!

It seems that the issue is somehow caused by the website layout.

First of all, try an iFrame version of the form on your web page to see if that works better.
