Account Suspended: Password Fields

    My account has been suspended. I can't log in. Please enable my account.

    Thank you for reaching out to Jotform Support. I checked your account and it has been suspended due to this form. Please take note that collecting login credential is not allowed in Jotform and is against our terms of use. In order to reactivate your account back, we'll have to remove/delete the login fields in the form.

    Let us know if you agree with our terms so we can go ahead and proceed in reinstating your account.

    Apologies for the back and forth but just wanted to confirm that we'll have to delete the related submission data with the offending field as well. Let us know if that sounds good to you so we can proceed.
